Overall Objectives

The basic objectives of all surgical sub-internships are the following:

1. To prepare the student for their upcoming PGY-1 year.

2. To expose the student to the clinical practice of surgery, specifically emphasizing the pre- and post-surgical care of inpatients. The student will function at the supervised PGY-1 level.

3. To further expound on the surgical knowledge base accumulated during the third-year clerkship.

4. To foster interest and/or further stimulate students considering careers in surgery or surgical subspecialties.

Description

The surgical sub-internship (Sub-I) is a focused month designed to prepare students for upcoming residency. Students will directly participate in the care of surgical patients; that is, initial evaluation, formulation of differential diagnoses, establishment of treatment plans and eventual daily care of the patient. It is important to emphasize that the student should function at the PGY-1 level with close supervision from the resident and attending surgical team. Students will actively participate on rounds, daily care tasks, procedures, and conferences.

The Sub-I is a chance for the soon-to-be resident to gain and/or polish the clinical skills critical for entering residency. The student should actively participate in procedures, clinical decision-making, and patient interaction. Call will be generally every fourth night. This month is a chance to establish a preliminary comfort level in taking care of patients and is designed to simulate what it may be like as a PGY-1, with appropriate supervision. Although the patient population is primarily of a surgical nature, students will obtain a broad clinical exposure which will provide a solid experience for entrance into any type of residency program.

The Sub-I is currently available on four surgical services at two sites:

Stony Brook University Hospital
l Breast Surgery
l General Surgery
l Plastic Surgery
l Surgical Intensive Care Unit
l Surgical Oncology
l Trauma Surgery (days)
l Trauma Surgery (nights)
l Vascular Surgery

Winthrop–University Hospital
l General Surgery
l Surgical Intensive Care Unit

Evaluation

Grading for the Sub-I in surgery is based on clinical performance and presentations given during the course of some of the sections. Performance on rounds, teaching conferences and interactions with the team are used to base decisions on the grade of a student. Both attendings and residents are involved in the grading process.

Daily attendance is mandatory for the month. Requests for leave of absence should be sought through Dr. Merriam's office.
