Re-activation Request for Year 2008

Re-activation of a prior application


All applicants to M.S., M.S.W., Post M.S., DPT or Health Care Policy and Management Certificate programs can request re-activation of a prior application. Requirements are that the re-activated application had to have been submitted for the prior academic year. For example, if the current application year is '2008', then the prior submitted application must be for '2007'. Also, requests for re-activation can be done only once without incurring any additional application fee; i.e. if your application was already re-activated a previous time, you must submit a new application including the $60 application fee, payable to 'Stony Brook University'.

Please Note: Applicants to the MPH program who wish to re-activate a prior application must contact the Graduate Program in Public Health directly at (631) 444-2074.

If you do wish your prior year application to be considered for re-activation, please download and complete the appropriate Re-activation request form.
Mail completed form to:
Office of Student Services
Health Sciences Center, L2 - Rm 271
Stony Brook, N.Y. 11794-8276


Upon receipt of this form, your eligibility to re-activate your application will be evaluated and a decision will be made. The 'Re-activation form' is a 'turn-around document'; i.e. you will receive this form in the mail with the re-activate application decision noted on the bottom. If approved, please download, complete and submit the following additional forms to our office: For Social Work - Pathway I applicants only - you need to download, complete and submit this additional form: Advanced Standing Practicum Evaluation
If there are any changes in your education, you must also submit updated official transcripts.
