Stony Brook University Health Sciences Center application procedure places responsibility on the applicant for gathering supporting credentials and submitting them to the Office of Student Services. Preferrably, you should mail them in one envelope (but not required). This will allow Stony Brook University Health Sciences Center to process your application in a timely fashion. Some institutions will only mail official transcripts directly to our office. That is acceptable.
All supporting materials should be mailed as soon as possible (you do NOT have to wait to submit your on-line application before you mail your supporting documents).

Transcripts (all applicants)
Request official college transcripts be sent to you or our office from ALL current and/or previous colleges and universities attended. This applies even if the credit is transferred in another transcript or not used toward the program that you are applying to. If the transcript(s) are mailed to you, DO NOT OPEN THE ENVELOPE. Insert the unopened transcript(s) and any other materials (references, etc.) into an envelope and mail to to the Office of Student Services. To expedite the review of your application, an unofficial transcript may be submitted. However, the official transcript including your fall semester grades (in the year of application) is required. The Office of Student Services is unable to provide copies of submitted transcripts.
Exceptions:- Current 'Matriculated' Stony Brook students do not have to request official transcripts from Stony Brook University or their other previous colleges.
- Former Stony Brook students may not have to request official transcripts from Stony Brook university (this will assessed when your application is submitted).
Reference Reports (all applicants)
A complete application requires three Reference Reports. You can choose to use either the on-line reference system or paper copy.
On-line Reference: When you submit your application, you will be able to use the on-line reference. Just fill in the appropriate information (you will need the email address of your reference person). The system will then send a copy of the reference form to them at the email address you indicate. When they complete the form and click 'send', it will be sent back to our system and included with your application.
(The on-line reference system is available only in the Schools of Nursing, Social Welfare and Health Technology and Management)
For Paper Copy: Please download and print the official Reference Report from the on-line application (available under 'Downloadable Forms' section) or select the appropriate reference form below. Fill out the top of each of the Reference Reports and give to your reference person with a self-addressed envelope. After the reference person has completed the report, it should be placed in a sealed envelope and signed across the seal by the person completing the form before it is returned to you or mailed to our office. When the references are received, DO NOT OPEN THE ENVELOPES. Insert the unopened references and any other materials (transcripts, etc.) into an envelope. Faxed Reference Reports or letters are not acceptable. Please remind your reference person of the appropriate deadline for returning the completed and sealed reference to you. Please ask them to keep a copy of their completed reference.
The law requires that letters of recommendation, reference or evaluation be kept confidential if the applicant waived all subsequent rights to examine them. Before sending your Reference Report to the person filling it out, please indicate your choice in the waiver box and sign each Reference Report. Unsigned waivers will not be accepted.
