Application Instructions and Applying On-line

Applications for admission to programs in the Schools of Health Technology and Management, Nursing, Social Welfare as well as the Graduate Program in Public Health are available through our on-line system only. Paper applications are not available. Application information for the School of Dental Medicine and the School of Medicine can be obtained by contacting each school.

What Makes a Complete Application
A complete application to the Health Sciences Center, Stony Brook University includes:
  • Submission of the on-line application.
  • Payment of Application Fee.
  • Three references submitted on the appropriate official Reference Report form. References should be solicited from former professors, employers, supervisors or health care professionals. References from personal friends or relatives are not acceptable.**
  • Official transcripts from all institutions attended (including institutions that granted college credit while you attended high school; and those that granted credit for courses not relevant to intended program). To expedite review of your application, unofficial transcripts may be submitted until your official transcripts arrive. Current and former Stony Brook students may not have to request official transcripts from Stony Brook University. (This will be assessed when your application is submitted.)
Stony Brook University's Health Sciences Center application procedure places responsibility on the applicant for requesting supporting credentials and submitting them to the Office of Student Services. Any additional documentation required by the program you are applying to should be mailed directly to the Office of Student Services. This will allow Stony Brook University, HSC to process your application in a timely fashion. Mail all documentation to:

The application for admission, together with all supporting documents, becomes the property of the Health Sciences Center, Office of Student Services once it is submitted. No materials will be returned to the applicant or sent to other institutions. We recommend that you make copies for your own records. All admission correspondence and decision letters will be mailed to your permanent home address.
You will receive immediate acknowledgement of your application submission to your email address. Instructions will be included so that you may track your application documents on-line using Stony Brook’s SOLAR system.
*NOTE: The law requires that after January 1, 1975, letters of recommendation, references or evaluations, may be kept confidential if the applicant has waived all subsequent rights to examine them. Since confidentiality may affect the type of reference given, the applicant may prefer to sign the waiver. Be sure to check the box indicated on the Reference Report included in the application.

Application Submission Deadlines

All programs have individual prerequisites and deadline dates which vary. You will not be able to submit the on-line application after the deadline has expired. Therefore, please check the "Individual Program Deadline Dates" to make sure that you will have enough time to complete the application.

Application Fees and Waivers

All applicants are required to pay a Stony Brook University Application Fee. This fee is $40 for each undergraduate program as well as all non credit programs. Graduate and Advanced Certificate programs are $60. Payment can be made by check or credit card (MasterCard, Visa or American Express are accepted). The Application Fee can be waived in some cases as listed below:

Types of Waivers:
  • Graduate, DPT, PA Post-Professional or Advanced Certificate Applicants
    • Academic - Currently enrolled undergraduate students in a U.S. institution who hold a cumulative GPA of 3.75. To qualify, the waiver request must be submitted with the application for admission, including official copies of all transcripts.
    • Financial - Students who are U.S. citizens and who have current documentation from a financial-aid administrator or other appropriate college or university official, substantiating that they are currently enrolled as an undergraduate and that payment of the application fee would create a financial hardship. Complete financial information for the current academic year must be provided, including total cost of education and the amount and types of financial-aid received.
    • Students who reapply for admission within one academic year. Reactivation forms are available at http://www.uhmc.sunysb.edu/studserv/reactivate.html
  • Undergraduate Applicants
    • Transfer applicants who are enrolled in the Educational Opportunity Program (EOP, HEOP, SEEK, College Discovery). A letter from the EOP program director confirming current enrollment in the program is required to process this waiver.
    • Students who are graduating from SUNY and CUNY two-year colleges and are applying for the next academic term(excluding summer). Official transcript indicating graduate degree and degree date are required.
    • Currently enrolled CAS or CEAS Stony Brook students in a matriculated, undergraduate program (not graduating) may also apply for a waiver.
If requesting a fee waiver, all waiver documentation must be mailed to the HSC Office of Student Services and will be assessed to determine if a fee waiver will be granted. If the fee waiver is not granted, you will be notified to submit the required application fee so your application can be processed.
