Course Locations

BRENTWOOD HEALTH CARE CENTER 

Nichols Road South to L.I. Expressway. LIE West to Exit 56 (Rt 111) South.  Bear right at the fork in the road and continue on #111 to Suffolk Avenue.  Right turn on Suffolk (West) until Brentwood Road.  Left on Brentwood. Go over RR tracks and make a left into the shopping center (Checkers, Genovese and Compare Foods). Health Center is inside the shopping center.

ELSIE OWENS N. BROOKHAVEN
AT CORAM HEALTH CENTER

3600 Rte. 112, Coram. Nichols Road South to Route 25 (Jericho Turnpike).  East on 25 to Route 112.  South again on Route 112 for approximately one mile.

DOLAN FAMILY HEALTH CARE  CENTER

Take expressway to exit 51 North  (route 231) Deer Park Road.  Go North to Huntington and Northport which is about 5 minutes.  Cross under Northern State Parkway which comes to a fork.  There is a fire station on left side.  Take left fork towards Huntington.  Follow Deer Park Road, cross over Jericho.  Go Park Avenue North till Pulaski Road, make right on Pulaski and turn left on 284 Pulaski, which is Dolan Family Health Center.

GLEN COVE HOSPITAL

LIE to Exit 39 North - follow Glen Cove Road North approximately 7 miles - Glen Cove Road becomes Cedar Swamp Road - Cedar Swamp to right on Pearsall (church on right/clam house on left).  Follow to hospital - turn left on St. Andrews one block turn left onto Trubee Place - parking lot on left, entrance to Family Practice.  Any problems 676-5000 ext. 2630.

NASSAU COUNTY MEDICAL CENTER

Directions:  From the west, take Long Island Expressway (Rte. 495) to exit 38, Northern State Pkwy.  Take exit 31A to the Meadowbrook Pkwy.  Exit at M5 east, Hempstead Tpke.  NCMC will be on your left in approximately one mile. From the east, take Northern State Parkway to exit 33, Wantagh State Pkwy.  Take exit W3 Hempstead Tnpk west.  The hospital will be on your right.

NORTHPORT VA MEDICAL CENTER

Directions:  From LIE; Take exit 53 or the Northern State Pkwy exit 45 to the Sunken Meadow Pkwy north to Route 25A West (not Route 25 West). This is the last exit, the Huntington exit. Route 25A west for 3 ½ miles. Make a left hand turn onto Rinaldo Road opposite the Salonga gas station. Rinaldo Road dead ends directly onto the golf course at the VAMC.

SOUTH NASSAU COMMUNITIES HOSPITAL

Route 347 to Northern State Parkway.  Northern State (West) to Sagtikos Parkway South.  Exit to Southern State Parkway West.  Exit 19 (Village Avenue) South to Merrick Road.  East on Merrick Road to Washington Avenue, make a right on Washington and the hospital is on the right.

STONY BROOK PRIMARY CARE CENTER

Take Nichols Road south to Rte. 347. Make a left onto 347 and drive east about 2 miles to Belle Meade Rd. Make a left onto Belle Meade Rd. Make a right into the parking lot for Building 205. The Primary Care center is on the North side of the building.

WINTHROP UNIVERSITY HOSPITAL

Take Northern State Parkway to Meadowbrook Parkway South (second exit) to Old Country Road West.  Go to Mineola Blvd (right turn) up 2 blocks cross railroad tracks.  Make left turn at intersection and hospital is right there.
