VI. Regarding Affiliate Agreements...

Occasionally, although rarely,  a site will ask that affiliate agreements be updated or will cancel an agreement. It is the student's responsibility to check with the Office of Medical Education as to the status of any affiliate agreement even if the site appears on the Website list when the student applies for the elective. Sites have the option to request an updated agreement and are most likely to do so upon receiving the Elective Approval form for signature. Updating agreements can take days or months. Please always plan a backup and always plan well ahead of time.

If you need extra forms - you can print them here.
