SPARC
Consortium Agreement
The members
of the SPARC Consortium are joined together by their commitment to the
delivery of comprehensive, coordinated family-centered services to HIV
infected and affected women, children and adolescents in Suffolk County.
Each agency
signing this Consortium Agreement agrees to work with other organizations,
and HIV infected and affected women and teens, to accomplish the following:

identify
health and social service needs of HIV affected women, children and adolescents;

develop an
active referral network to improve coordination of services;

assign representative(s)
to act as liaison between SPARC and agency (to disseminate appropriate
information to staff who do not attend SPARC meetings, and to provide relevant
agency information regarding services and needs to other SPARC
Consortium members);

attend meetings
of the SPARC Consortium - anticipated to be held twice a year (agencies
represented on the Steering Council are anticipated to meet quarterly).
If a designated representative is unable to attend a meeting, a delegate
should be assigned to take her/his place;

participate
in subcommittes formed address specific service gaps/barriers/etc. and
develop strategies to resolve these issues;

promote full
access to existing community-based resources and advocate for the creation
of additional services to fill unmet needs;

incorporate
the provision of risk reduction information and education into comprehensive
care programs (including both information of theose at risk for HIV infection
and information on the maintenance of health for those living with HIV/AIDS);

work collaboratively
with other agencies to develop new programs to meet identified needs (possibly
including the submission of joint grant proprosals).
